Ayushman Bharat Card 2025
What You'll Learn
- How PM-JAY eligibility is identified for rural and urban households
- What the 70 years and above Ayushman Vay Vandana expansion changes
- How to check beneficiary status and obtain an e-card through official channels
- Which hospital, package, document and grievance checks should be completed before treatment
Ayushman Bharat Card 2026 is a common search phrase for two related tasks. A person may want to know whether a family is entitled under Ayushman Bharat Pradhan Mantri Jan Arogya Yojana, or may want to create or download the e-card used at an empanelled hospital. Eligibility is not created merely by searching the portal. The live beneficiary record, identity verification, state implementation and hospital package rules matter.
Ayushman Bharat Card 2026: Key Facts and Current Scope
Ayushman Bharat PM-JAY is implemented by the National Health Authority under the Ministry of Health and Family Welfare. The myScheme description says it provides cashless hospitalisation coverage of up to Rs. 5,00,000 per family per year for poor and vulnerable families for secondary and tertiary healthcare. The scheme was launched on 23 September 2018 in Ranchi, Jharkhand.
The coverage is for identified packages at public and empanelled private hospitals. The official description includes consultation, medicines, diagnostics, surgery, ICU, implants, accommodation and food within covered treatment packages. It also lists 3 days of pre-hospitalisation and 15 days of post-hospitalisation support. These statements should not be read as a promise that every outpatient expense, medicine or hospital service is covered.
| Feature | Official description | What the reader should verify |
|---|---|---|
| Annual cover | Up to Rs. 5,00,000 per family per year | Beneficiary record, package eligibility and available state rules |
| Care type | Secondary and tertiary hospitalisation | Whether the planned treatment is an identified package |
| Hospital access | Public and empanelled private hospitals | Whether the selected hospital is currently empanelled |
| Pre and post support | 3 days before and 15 days after hospitalisation in the listed benefit description | Package terms and documents required for the claim |
| Pre-existing conditions | Covered from day one in the listed PM-JAY description | Package rules, clinical records and hospital confirmation |
The NHA beneficiary portal The NHA beneficiary portal provides a route to check beneficiary status, card status and other self-service features. The official portal is the correct place to start. A private website may explain the process, but it cannot create an entitlement or replace the government record.
Our Mahatari Vandana Yojana guide also separates scheme eligibility from general benefit claims. The same discipline applies to PM-JAY.
Who Can Be Eligible Under PM-JAY
For the main PM-JAY population, myScheme describes an entitlement-based identification system using deprivation criteria for rural households and occupational categories for urban households. It says there is no general enrollment process for families because eligible households are identified through government data. A person who is not found should use the official search and grievance routes rather than treating a social-media list as proof.
The page also describes family and individual identification through an Arogya Mitra or operator. The operator can search using name, location, ration card number, mobile number or an RSBY URN. If the record is found, identity and family documents are checked, uploaded and sent for approval according to the process described by the scheme.
Aadhaar needs careful explanation. The myScheme FAQ says Aadhaar is not mandatory for availing services under PM-JAY, while the identification process may use Aadhaar or another government ID together with a ration card or alternative family ID. Therefore, a missing Aadhaar number should not be presented as automatic disqualification, and a reader should ask the Arogya Mitra which alternative document is accepted in the current workflow.
Rural Eligibility Criteria to Check
The myScheme page lists six rural deprivation criteria and additional automatic-inclusion categories. The criteria are household indicators from the scheme's identification framework. They are not a substitute for a live beneficiary search. A family should check its record through the official portal, helpline or an authorised help point before concluding that it is or is not eligible.
| Rural indicator listed by myScheme | How to read it | Verification point |
|---|---|---|
| Only one room with kucha walls and kucha roof | A housing deprivation indicator | Confirm the family record rather than relying on a self-assessment |
| No adult member between ages 16 and 59 | An age and household-composition indicator | Check names and ages in the government record |
| No adult male member between ages 16 and 59 | A household-composition indicator | Ask the help point how the record is matched |
| Disabled member and no able-bodied adult member | A disability and support-capacity indicator | Carry accepted identity and family documents |
| SC or ST household | A listed social-category indicator | Use the current beneficiary search and state process |
| Landless household with major income from manual casual labour | A livelihood indicator | Do not infer eligibility from occupation alone without a record match |
| Automatic-inclusion categories | Includes households without shelter, destitute or alms-dependent households, manual scavenger families, primitive tribal groups and legally released bonded labour | Seek assisted verification through an official channel |
The page also lists exclusion indicators such as ownership of a two-, three- or four-wheeler or a motorised fishing boat, mechanised farming equipment, a Kisan card with a credit limit above Rs. 50,000, government employment, monthly income above Rs. 10,000, a solidly built house and land ownership of 5 acres or more. Because these are source-listed indicators, not a personal eligibility assessment, readers should verify how the current state database applies them.
Urban Occupational Categories Under PM-JAY
For urban areas, the myScheme page lists 11 occupational categories. These include ragpickers, beggars, domestic workers, street vendors and related service providers, construction workers and related labour, sweepers and sanitation workers, home-based workers and artisans, transport workers, shop workers and assistants, electricians and mechanics, and washer-men or chowkidars.
An occupation list is not a card by itself. The family must still be identified in the eligible database and complete the verification process. If a worker has changed jobs, moved location or uses a different spelling of the name, the applicant should ask an authorised operator how the record can be matched without submitting inconsistent documents.
Our Shram Shakti Portal article discusses why worker registration and health-benefit eligibility should not be treated as the same database. A registration on another portal does not automatically create PM-JAY cover.
Senior Citizens Aged 70 and Above: Ayushman Vay Vandana
The 2024 expansion described in the PIB release covers all senior citizens aged 70 years and above under Ayushman Bharat PM-JAY, irrespective of socio-economic status. The senior-citizen benefit is represented through the Ayushman Vay Vandana Card. A senior from a family already covered under PM-JAY receives an additional top-up cover up to Rs. 5 lakh per year for the senior person under the PIB description.
The senior expansion has a separate coordination issue for people already covered by the Central Government Health Scheme, Ex-Servicemen Contributory Health Scheme or Ayushman Central Armed Police Force. The PIB release says those individuals have to choose between their existing scheme and AB PM-JAY. It also says people with private health insurance or Employees' State Insurance can be eligible. This is not advice to cancel an existing policy. The person should compare the applicable rules and confirm the choice before treatment.
The PIB release says eligible seniors can register at an empanelled hospital, through the Ayushman app or beneficiary.nha.gov.in, by calling 14555 or by giving a missed call on 1800110770. It describes coverage for around 2,000 medical procedures and pre-existing diseases from the first day without a waiting period. These senior-specific descriptions should be kept separate from the general PM-JAY procedure count shown on myScheme.
What the Ayushman Card Covers
The card is evidence of a beneficiary record connected to an entitlement. It is not an unlimited reimbursement card. The myScheme page says PM-JAY provides cashless access for identified secondary and tertiary hospitalisation packages at public and empanelled private hospitals. It lists treatment elements such as consultation, medicines, diagnostics, surgery, ICU, implants, accommodation and food when they fall within the covered package.
MyScheme states that approximately 1,929 procedures are listed for the scheme. The PIB senior-citizen release separately refers to around 2,000 medical procedures for the Ayushman Vay Vandana context. The difference may reflect the specific context or a later source presentation. Readers should use the current package list and hospital confirmation for the treatment they need, rather than relying on one headline number.
| Possible treatment element | Official source treatment | Before admission |
|---|---|---|
| Consultation and diagnostics | Listed within the myScheme benefit description | Ask whether they are part of the selected package |
| Medicines and surgery | Listed within covered treatment costs | Confirm package inclusion and pre-authorisation requirements |
| ICU and implants | Listed within the benefit description | Confirm clinical and package conditions with the hospital |
| Accommodation and food | Listed within the benefit description | Ask the Ayushman Mitra what the package provides |
| Pre and post hospitalisation | 3 days before and 15 days after in the listed description | Retain prescriptions, reports, discharge papers and bills where relevant |
The official description also says all pre-existing diseases are covered from day one. That does not mean every medical service is covered without a package or that a hospital can skip its normal clinical and approval process. The hospital's Ayushman Mitra should explain the package and the documents required for the specific admission.
For another government-service process, see our Adi Vaani guide. It is an internal navigation link and not evidence for PM-JAY rules.
What the Card May Not Cover
Readers should not assume that the Ayushman card pays for every outpatient consultation, every medicine purchased outside a hospital package, every facility or every personal expense. The scheme description is centred on secondary and tertiary hospitalisation and identified packages. A hospital may also need to complete package verification or pre-authorisation before treatment.
Portability does not remove the need to use an empanelled hospital. The NHA portal provides a hospital-search feature, and the myScheme page says services are available at empanelled public and private facilities. Check the hospital list and package availability before travelling. A search result on an old page or a private directory is not a current empanelment certificate.
Do not confuse the Ayushman card with an ABHA digital-health identity. The two services have different purposes. The PM-JAY beneficiary record relates to health-assurance eligibility and covered packages, while ABHA is part of the digital-health ecosystem. The presence of one does not prove entitlement to the other.
Our Yogyashree Scheme guide is another example of why one government programme's eligibility cannot be transferred to a different programme.
How to Check Ayushman Beneficiary Status Online
Open the NHA beneficiary portal and use the beneficiary login with a mobile number. The portal says an existing beneficiary can also use a member ID for self-service. Its listed features include linking Aadhaar, adding a member to an existing family, redoing e-KYC to update a photo or address, checking card status and finding empanelled hospitals.
Use the live result as a status check, not as a promise of approval. If the record is missing, verify the spelling of the name, mobile number, family details and state selection. Then contact an authorised help point or the 14555 call centre. Do not share an OTP with an unauthorised person or pay a middleman for a benefit that the official process describes as cashless for eligible beneficiaries.
The portal is the relevant channel for a current record, while myScheme provides the broader scheme description. The myScheme page displayed a last-updated date of 20 August 2026 during research, and the NHA beneficiary portal displayed 25 March 2026. Web pages can change, so the live portal and current state instructions should control a real application.
How to Get the E-Card Through a CSC or Hospital
The myScheme application description says an Arogya Mitra or operator can search available beneficiary lists using a name, location, ration card number, mobile number or RSBY URN. If the individual is found, identification uses Aadhaar or another government ID and a ration card or alternative family ID. Scanned documents are uploaded as part of the process.
The described flow then identifies the family, submits the record to the trust or insurance company for approval, and sends cases recommended for rejection to the State Health Agency for final verification. The e-card is issued after approval. This is why a person should keep the acknowledgement or status reference and should not treat an unverified download link as a valid card.
For a senior aged 70 years and above, the PIB release provides additional routes through the Ayushman app, beneficiary.nha.gov.in, an empanelled hospital, 14555 or the 1800110770 missed-call number. Use the route that is available and keep the beneficiary's name and date-of-birth details consistent across documents.
How to Use the Card at an Empanelled Hospital
Start by checking whether the hospital is empanelled and whether it offers the relevant package. At the hospital, ask for the Ayushman Mitra or the designated help desk. The official scheme description identifies the Ayushman Mitra as a frontline contact who assists with beneficiary identification and claim processing.
Carry the card or beneficiary details, identity documents, family documents, prescriptions and previous clinical records where relevant. Ask what is included before signing for a package or paying an amount. If a facility asks for money for a service that appears to be part of an approved cashless package, request a written explanation and contact 14555 or the state agency.
Do not publish or share another person's health documents. A beneficiary may need to show identity and clinical papers to an authorised hospital, but privacy and consent remain important. The card should not be treated as permission for a third party to access a patient's entire medical history.
Documents and Common Verification Issues
The myScheme page lists age and identity proof, address proof, contact details, caste certificate, income certificate, family-status documents and Aadhaar among its document examples. The identification process also refers to a ration card or alternative family ID. The exact combination can depend on the beneficiary record and the state workflow, so the list should be treated as a preparation guide rather than an inflexible universal rule.
| Check before visiting | Documents or evidence to keep | Why it helps |
|---|---|---|
| Identity and age | Government identity proof and age evidence | Helps match the beneficiary record and senior-citizen age where relevant |
| Family identity | Ration card or accepted alternative family ID | Supports family-member identification |
| Address and contact | Address proof and active mobile number | Supports portal login and record communication |
| Category or income record | Caste or income document if requested by the current workflow | Helps resolve a data mismatch without submitting contradictory details |
| Treatment evidence | Prescription, reports, discharge summary and package papers | Helps the hospital assess the treatment and claim documents |
Common problems include a name mismatch, a family member missing from the record, a changed mobile number, incomplete e-KYC, a hospital that is not currently empanelled and confusion between ABHA and PM-JAY. Record the date, location, operator name or help-desk reference when asking for correction. Escalate through 14555 or the relevant state health agency if the issue is not resolved.
Portability, Grievance Help and a Practical Checklist
MyScheme describes nationwide portability at empanelled hospitals. Portability means a beneficiary can seek covered services outside the home location at an empanelled facility, subject to package and operational rules. It does not mean that every hospital or every treatment becomes free. Confirm the hospital, package and approval route before admission.
For a complaint, begin with the hospital Ayushman Mitra or official help desk. The official description provides the helpline 14555 and refers to district, state and national grievance committees. Keep written records of the issue, the beneficiary ID, hospital details, dates, receipts and any response. Do not pay an unauthorised agent to file a grievance.
- Check the beneficiary record on the NHA portal or through an authorised operator.
- Confirm the hospital's current empanelment and the relevant treatment package.
- Keep identity, family, age, address and treatment records consistent.
- Ask the Ayushman Mitra about cashless package terms before admission.
- Use 14555 for official help and keep the complaint reference.
- For a person aged 70 years and above, check the Ayushman Vay Vandana route and scheme-choice rules.
Eligibility and treatment rules can be updated. The safe approach is to use this article as a preparation guide, then confirm the live result, hospital and package through the official channel on the day the information is needed.
